Your Community Health is dedicated to creating healthier communities and improving the health and well-being of individuals. It is a non-profit organization aiming to expand its reach and provide access to premium quality healthcare regardless of their background and financial condition.
Your Community Health emphasises on enhancing the health and well-being of the community through education, care, and support. It follows a holistic approach where the services are catered according to the necessities and requirements of the individual. It believes in providing access to healthcare services to everybody and that healthcare is a fundamental human right.

- National Disability Insurance Scheme (NDIS): NDIS is the new way of funding and providing disability support for individuals who are under 65 years old and have a significant or permanent disability. NDIS participants can access several NDIS-funded services, such as Physiotherapy, Occupational Therapy, Dietetics and Nutrition, Podiatry, Group and center-based activities.
- Dental Services: The professional dental team at Your Community Health provides high quality treatment to prevent oral health and teeth problems. It provides free or low-cost public dental services. It also provides free or low-cost emergency and general care for children.
- Medical Services: The expert team at Your Community Health assists patients by ensuring regular health checkups. With a diverse range of specialist medical services, Your Community Health prioritises individuals seeking urgent medical attention.

- Allied Health Services: Your Community Health offers tailored services according to the requirements of patients for all stages of life. Some of the allied health services are Alcohol and other drugs, Diabetes education, Gambler’s Help, Mental Health, Occupational Therapy, Optometry, Podiatry, Speech Therapy, and so on.
- Social Support Groups and Activities: The diverse range of these services are Crafts and games, Gentle exercise and movement to music, Spiritual activities and reflection, and Outings and day trips. Your Community Health also organises interactive and fun programs, such as Kookaburra Club, Ladies’ Social Groups, and The Darebin Men’s Shed.
- Aboriginal and Torres Strait Islander Health: Your Community Health’s aboriginal health team ensures a culturally safe space for members of the community. It is committed to provide support the community in all areas of health and wellbeing. The aboriginal and Torres Strait islander community members are given priority in these services.
- Aged Care services: These services are catered to individuals who are 65 years and older or an Aboriginal and/or Torres Strait Islander who is 50 years and older. These services are either provided at Your Community Health clinic or at the recipient’s home.
- Trans and Gender Diverse in Community Health Program: Your Community Health offers healthcare and gender affirmation services to trans, gender diverse and non-binary Victorians, individuals with sex variations, and those seeking endocrine assisted medial affirmation support.
- Refugee and Asylum Seeker Health: Your Community Health offers special medical services for the newly arrived refugee and asylum seekers. Several Refuging Nursing Services are offered at Your Community Health.
